When not to use DexaPico
– if you are allergic to lime blossom or dextromethorphan hydrobromide or any of the other ingredients of this medicine;
– if you have a cough with copious discharge;
– in patients with bronchial asthma;
– in patients with severe liver disease;
– in patients with respiratory failure or at risk of its occurrence;
– if you are taking monoamine oxidase (MAO) inhibitors at the same time and within 14 days after stopping them;
– in patients taking selective serotonin reuptake inhibitors (SSRIs);
– in patients taking mucolytic drugs (thinning of mucus in the airways).
Warnings and Precautions
Talk to your doctor or pharmacist before taking DexaPico.
Use of this product may lead to addiction. Therefore, treatment should be short-term.
If you are taking medications such as certain antidepressants or antipsychotics, DexaPico may interact with them, causing mental status changes (eg, agitation, hallucinations, coma) and other symptoms such as body temperature above 38°C, increased heart rate, unstable blood pressure , as well as exaggerated reflexes, muscle rigidity, incoordination, and/or digestive symptoms (eg, nausea, vomiting, diarrhea).
If you have a chronic or persistent cough, check with your doctor before using the medicine. If symptoms persist for more than 7 days, the patient relapses

Do not exceed recommended doses.
There have been reports of abuse of drugs containing dextromethorphan. Particular caution is advised when using this medication in adolescents and young adults, and in patients with a history of drug or substance abuse.
The use of dextromethorphan hydrobromide has not been specifically studied in patients with renal or hepatic impairment.
Because of the extensive hepatic metabolism of dextromethorphan hydrobromide, caution should be exercised in patients with hepatic impairment.
Caution should be exercised in patients with poor CYP2D6 metabolism or concomitant use of CYP2D6 inhibitors due to the potential increase and/or long-term effects of dextromethorphan.
Children
The use of the drug in children under the age of 2 years is not recommended due to the lack of studies on the use of the drug in infants and young children.
For children from 2 to 12 years old, the drug is recommended to be administered only on the recommendation of a doctor.
DexaPico and other medicines
Tell your doctor or pharmacist about all medicines you are taking, have recently taken or plan to use.
MAO inhibitors
The drug should not be used in patients taking MAO inhibitors, and within 14 days after stopping treatment with MAO inhibitors due to serotonin syndrome (high
fever, hypertension, arrhythmias). Serious adverse reactions, and sometimes death, have been reported with the use of dextromethorphan hydrobromide in patients treated with MAO inhibitors.
Antibacterial drugs
Linezolid: serotonin syndrome has been observed when co-administered with dextromethorphan hydrobromide.
CYP2D6 inhibitors
The simultaneous use of strong inhibitors of the CYP2D6 enzyme can increase the concentration of dextromethorphan in the body to levels many times higher than normal. It increases it
risk of dextromethorphan toxicity (agitation, confusion, tremor, insomnia, diarrhea and respiratory depression) and the development of serotonin syndrome. Strong inhibitors of the CYP2D6 enzyme include fluoxetine, paroxetine, quinidine, and terbinafine. With simultaneous use with quinidine, the concentration of dextromethorphan in plasma can increase up to 20 times, which increases the risk of side effects from the central nervous system associated with the use of this drug. Dextromethorphan is also caused by amiodarone, flecainide and propafenone, sertraline, bupropion, methadone, cinacalcet, haloperidol, perphenazine and thioridazine. If concomitant use of CYP2D6 inhibitors and dextromethorphan is required, the patient should be monitored. You may also need to reduce the dose of dextromethorphan.
Due to the content of dextromethorphan hydrobromide, the simultaneous use of the drug with alcohol or other drugs that depress the central nervous system may increase their inhibitory effect on the central nervous system and be toxic in relatively small doses.
DexaPico with food, drink and alcohol
During treatment, it is not recommended to drink alcohol.
Pregnancy and lactation
If you are pregnant or breastfeeding, think you may be pregnant, or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
Do not use for pregnant and lactating women.
